*Registration Packets available beginning April 2nd at District Office & Elementary buildings*
- Families should register at the home school
- Child must be 5 years old on or before September 1, 2018 (School Code 105 ILCS 5/10-20.12)
- To complete registration, the following must be provided:
- Original birth certificate
- Immunization records
- Lease/Mortgage
- (2) Utility Bills
- Parents residing at the home of another? Follow Proof of Residency Form Instructions on page 2
- Families requiring a Spanish-speaking registrar may register at the Welcome Center in the District Administration Center at 1500 Sangamon, Crete, Illinois.
